  • Before completing the Georgia MV-6 form, read the instructions specific to the tag category you are requesting.

  • Provide a permanent twelve-digit Customer ID number and a current master tag number on the form.

  • Ensure to attach copies of required documents, such as the Georgia Tax ID number and business license, as specified for your tag category.

  • Check the appropriate box to indicate the tag category you are applying for. A separate MV-6 application is needed for each category or business location.

  • Submit all fees with a check or money order made out to the Department of Revenue. Cash should not be sent by mail.

  • Remember to sign and date the application. The authorized person must also provide their printed name and position within the company.
